• There is a parking garage on Oak Street in the Kaufman Rossin building. They will valet your car for free and we will validate parking 50%

Using Insurance?

I’m in network with most Aetna, BCBS (Florida and out-of-state plans), Cigna, and Medicare.

The rates listed on this page are self-pay rates. If we’re billing your insurance, your actual out-of-pocket cost — copay, coinsurance, or deductible — is set by your specific plan, not by these rates.

Once you book and complete your intake forms, I’ll verify your eligibility and send you an estimate of your coverage before your visit, so there are no surprises.
